Supporting People Living with Heart Failure
We support people living with heart failure and their families and carers. A heart failure diagnosis can be overwhelming for the person affected and for those around them. We offer practical, person-centred support to help people understand their condition, manage day-to-day activities, and stay well at home.

Meet Our Founder
Amina Sarah Nantale, MSc, ACP, Independent Prescriber
Founder & Director
Amina is an experienced Advanced Clinical Practitioner (ACP), Independent Prescriber, and Heart Failure Clinical Nurse Specialist with over 18 years of clinical experience across the NHS, working in primary care, secondary care, and the independent healthcare sectors in UK.
She has dedicated more than 10 years to supporting people living with heart failure in community settings, helping patients achieve the best possible quality of life through evidence-based pharmacological treatments, education and personalized care.
Amina is passionate about improving patient outcomes by empowering people with the knowledge, confidence, and support they need to live well with heart failure. She believes education, early intervention, and compassionate care can transform lives, helping people remain independent and live safely in their own homes for longer—one of the key priorities of the NHS Long Term Plan.
Alongside her clinical work, Amina has served as the voluntary Treasurer of the Uganda Muslim Community UK (UMCUK Charity) since 2024, supporting the charity's financial governance and community initiatives.She has also delivered numerous community outreach health education workshops in London and internationally, reflecting her commitment to improving health literacy and empowering communities. Helping others is at the heart of everything she does, both professionally and through voluntary service.
