We support people living with heart failure and their families and carers. A heart failure diagnosis can be overwhelming for the person affected and for those around them. We offer practical, person-centred support to help people understand their condition, manage day-to-day activities, and stay well at home.

Meet Our Founder
Amina Sarah Nantale MSc, ACP, Independent Prescriber
Founder & Director
Amina is an experienced Advanced Clinical Practitioner, a Heart Failure Clinical Nurse Specialist with over 18 years' experience working across the NHS in primary and secondary care and private. She has over 10 years of specialist experience supporting people living with heart failure in community settings, and she is also an independent prescriber.
She is passionate about improving access to education, empowering patients and carers, and helping people live well with heart failure.
